Transition
We understand transitioning from Childrens Services can be daunting. Our aim is to provide a smooth transition to Adult Services liaising with all key professionals to facilitate a safe comfortable stress free move. The team will provide clarity and reassurance explaining processes in a transparent open manner to support the young person and family during this time.
Direct Payments and Personal Health Budgets
We support families with Direct Payments and Personal Health Budgets with the following services:
- Recruitment of Nurses and Carers along with emergency cover in times such as sickness and holidays
- Provision of Clinical Educators to provide bespoke training and updates
- Providing Advocacy Services to ensure the family are listened to at review meetings
- Providing Escort Services to attend meetings and reviews
- Provision of qualified Budget Management Officers to help manage budgets
- Provision of Prepaid Cards with Real Time Budget controls and reporting
