Community Clinical Care Ltd is a Nurse led specialist complex care provider of individualised holistic care for Children, Young People and Adults with Complex Health Needs and Disabilities. We have over 30 years’ experience of working in partnership with Continuing Care, Case Management, Solicitors and Local Authorities in supporting service users in their own home and other community settings such as schools, respite centre, playschemes and other short break provision.
